City of Chicago Announces Proof of Vaccination Requirements

On December 21, 2021 the City of Chicago announced Public Health Order No. 2021-2. Effective January 3rd, 2022 any individual 5 years of age or older to show proof that they are fully vaccinated against COVID-19 to dine indoors, visit gyms, or entertainment venues where food or drink are being served.
